Clock Hour Resources
Clock Hours Form Procedure: Click here to access this information as a PDF.
WAFLT will offer clock hours if a WAFLT board member or designated WAFLT representative is present at the event. If a designated representative is not present, then approval for clock hours must be requested through Clock Hour Chair Nelia Floyd (firstname.lastname@example.org) or via the WAFLT website.
1. WAFLT must be a sponsor or co-sponsor of the event and a WAFLT board member or WAFLT designated representative must be present. Please fill out the clock hours request form.
2. Once WAFLT has approved the offer clock hours for an event, WAFLT must be considered as co-sponsoring the event. The WAFLT logo, name and website link (waflt.net) must be present on any type of communication such as flyers or website posts. If applicable, include the WAFLT Facebook link, too.
3. At the event, make sure all the paperwork is complete and accurate:
a. Sign in sheet that includes the title of the event, date, time in time out and location.
Attendees must include their full name, email and signature. An attendee sign in sheet and session evaluation form will be sent to you upon process of your request.
b. Upon completion of the event, attendees seeking clock hours must fill out the evaluation form to be eligible for hours. WAFLT must receive an evaluation form per clock hour form submitted. It is best to have the attendees first turn in the evaluation form and then be given the clock hour form.
c. Have applicants fill out the clock hour form and instruct them to turn the form into their school district (or follow district procedures) and to always make a copy for their records. WAFLT will not have a copy of their clock hour form.
4. Send/email the sign in sheets and evaluations to Nelia Floyd within one week of the event. It is recommended that you keep a copy of the sign in sheet for future reference. Scan and email to Nelia Floyd at email@example.com.
